From Long Island

The Long Island Expressway

1. Take exit number 44 named "ROUTE -135" South, towards SYOSSET/SEAFORD.
2. Merge onto SEAFORD OYSTER BAY EXWY.
3. Take the OLD COUNTRY RD exit, exit number 10, towards PLAINVIEW/HICKSVILLE.
4. Turn LEFT onto OLD COUNTRY RD.
5. On your right, south side of the road across from hospital.
   875 Old Country Road Ste 300

Northern State Parkway

1. Take exit number 36a named "ROUTE -135" South, towards SEAFORD.
2. Take the OLD COUNTRY RD exit- exit number 10- toward PLAINVIEW/HICKSVILLE
3. Turn LEFT onto OLD COUNTRY RD.
4. On your right, south side of the road across from hospital.
   875 Old Country Road Ste 300

Southern State Parkway

1. Merge onto SEAFORD OYSTER BAY EXWY/NY-135 N via exit number 28A N toward SYOSSET.
2. Take the OLD COUNTRY RD exit- exit number 10- toward PLAINVIEW/HICKSVILLE.
3. On your right, south side of the road across from hospital. 875 Old Country Road Ste 300

 

From New York City:

Via Midtown Tunnel

1. Take the Midtown Tunnel which merges into I-495 West - Long Island Expressway.
2. Take exit number 44 named "ROUTE -135" South, towards SYOSSET/SEAFORD.
3. Merge onto SEAFORD OYSTER BAY EXWY.
4. Take the OLD COUNTRY RD exit, exit number 10, towards PLAINVIEW/HICKSVILLE.
5. Turn LEFT onto OLD COUNTRY RD.
6. After RR tracks, across from hospital, on your right, south side of the road. 0.1 miles

Via the Triboro Bridge:

1. Upon leaving the Triboro Bridge Follow signs to Long Island - Grand Central Parkway
2. Exit Grand Central Parkway East - Long Island
3. Grand Central Parkway becomes Northern State Parkway - Continue East to exit 36a
4. Exit number 36a - named "ROUTE -135" South, towards SEAFORD.
5. Take the OLD COUNTRY RD exit- exit number 10- toward PLAINVIEW / HICKSVILLE
6. Turn LEFT onto OLD COUNTRY RD.
7. On your right, south side of the road across from hospital.

From New Jersey:

1. Take the I-95 N/N J TURNPIKE NORTH exit. 1                 
2. Merge onto NEW JERSEY TURNPIKE N/I-95 N (Portions toll).      6.6 miles
3. Take the RT-3 exit- exit number 16E- toward LINCOLN TUNNEL.   0.5 miles
4. Merge onto I-495 E (Portions toll).      3.9 miles
5. Stay straight to go onto DYER AVE.      0.1 miles
6. Turn LEFT onto W 34TH ST.      1.3 miles
7. Turn LEFT onto TUNNEL APPROACH ST/QUEENS MIDTOWN TUNNEL APPROACH/I-495 E.     0.1 miles
